*** INSTRUCTIONS UPDATE (01/31/2000): We found out that some IEP are not converting properly from MS Word.  Download the instructions update to Page 10 (section 4) of the instructions for the process to correct this problem.

*** If you need to transfer data or annotations from one IEP (in Acrobat) to another because you have an outdated IEP, click HERE to download or view the instructions.

These are the Special Ed forms needed for the conferences.  You are now be able to fill out the forms online and print them out.  If you need to save the forms with the student's name, you must have the Adobe Acrobat package (not the reader).  Please contact Mike Horvath at 349-4756 about any questions on the purchase and/or distribution of the Adobe Acrobat package.

If you already have the Adobe Acrobat package (not the reader), you have to download the PDF form and then open it with Acrobat.  You can then fill out the form and save it under the student's name to be used over again.

NOTE:  You can download the PDF file by doing the following (PC or Mac):

PC:  right-click on link, then select 'Save target as' (Internet Explorer) or 'Save Link as' (Netscape)  and choose file destination

MAC: either option-click on link or click and hold on link, then select 'Download link to disk' (Internet Explorer), or 'Save link as' (Netscape), and choose file destination

** The Adobe Acrobat Reader is free from the Adobe Web Site. The Reader allows you to fill out (and print) the form online, but can not save any changes.   After downloading the Adobe Reader, run the setup file and restart your browser (Netscape or Internet Explorer).
